BECOME A MEMBER

 

PLAN offers its Services and Programs to PLAN members and their families. 

Those eligible for PLAN membership include:

• Adults over the age of 18 who have a severe mental illness or other cognitive impairment
• People living in Northeast Ohio, primarily Cuyahoga, Lake, Geauga, and Summit counties

 

(Members/their family pay a nominal Annual membership fee and fees for social work services.)

 

PLAN services are paid for by:

• Medicare, Part B         
• Private health insurance (check your outpatient mental health coverage) and
• Private pay. 
• PLAN does NOT accept Medicaid reimbursement.
